Candidates holding documents qualifying them for higher education, issued in Poland

I. SCANS REQUIRED AT THE REGISTRATION STAGE

  1. a CV including a description of the candidate’s planned research and a list of the candidate’s academic achievements to date (template available in the IRK system);

  2. an opinion issued by the prospective supervisor1 concerning the candidate’s planned research and academic achievements to date, together with an opinion of the Dean of the relevant faculty of the ÷ÈÓ°Ö±²¥ of ÷ÈÓ°Ö±²¥ regarding the possibility of carrying out the research project proposed by the candidate within the relevant unit of the ÷ÈÓ°Ö±²¥ of ÷ÈÓ°Ö±²¥ (template available in the IRK system);

  3. a handwritten-signed declaration by the candidate stating that they are not a doctoral student at another doctoral school, generated in the IRK system;

  4. a diploma of completion of a second-cycle programme or a long-cycle Master’s degree programme at a Polish higher education institution, or a certificate of completion of such studies containing the final grade to be stated in the diploma and information on the grading scale applicable at the institution2;

  5. a certificate confirming the grade average specified in the detailed admission rules, where the grade average constitutes an admission criterion;

  6. a transcript of records obtained during the course of study, or a diploma supplement to the diploma of completion of a second-cycle programme or a long-cycle Master’s degree programme;

  7. documents confirming the candidate’s academic output (academic achievements), qualifications, or other accomplishments, in particular: a copy of publications, language certificates, certificates confirming the award of an academic prize, completion of an internship, or membership in a student research club; where a document has been issued in a language other than Polish or English, the candidate shall also submit a copy of that document translated into Polish or English;

  8. a completed and signed candidate achievements form (generated in the IRK system).

II. PAPER DOCUMENTS REQUIRED AFTER THE ANNOUNCEMENT OF THE LIST OF CANDIDATES

A candidate entitled to submit documents shall provide to the relevant unit responsible for doctoral school admissions either the originals or copies certified as true copies of the originals by: the higher education institution from which the candidate graduated, a notary public, or a member of the admission committee.

At the same time, the candidate shall present for inspection:

  • an identity document,

  • a Polish Card (Karta Polaka) — in the case of a foreign national who holds this document.

  1. an application for admission to the doctoral school, signed by hand by the candidate (generated in the IRK system);

  2. a CV including a description of the candidate’s planned research and a list of the candidate’s academic achievements to date (template available in the IRK system);

  3. an opinion issued by the prospective supervisor1 concerning the candidate’s planned research and academic achievements to date, together with an opinion of the Dean of the relevant faculty of the ÷ÈÓ°Ö±²¥ of ÷ÈÓ°Ö±²¥ regarding the possibility of carrying out the research project proposed by the candidate within the relevant unit of the ÷ÈÓ°Ö±²¥ of ÷ÈÓ°Ö±²¥ (template available in the IRK system);

1ÌýIn the case of applying for admission to a doctoral school in more than one academic discipline, the candidate shall submit opinions from prospective supervisors representing the respective disciplines.

2ÌýGraduates of the ÷ÈÓ°Ö±²¥ of ÷ÈÓ°Ö±²¥ are exempt from the obligation to provide information on the grading scale.
